Ancillary Study Proposal:

• Download the Ancillary Study Proposal form to request approval for collection of new data - or to propose analyses of existing data as part of a new funding application. See more on the definition of an ancillary study here.

Analysis-only studies: please complete part 1 ONLY of the Ancillary Study Proposal form prior to your grant application. Submit it as an uploaded supplemental file in your corresponding first paper proposal (online link below). See also: step-by-step instructions.

Manuscript Proposal:

• Submit a new paper proposal via the online manuscript proposal template.
• Update a previously submitted proposal (go to MyCHS). To learn more about modifications, click here.

Penultimate Draft Manuscript:

• Edit a standard penultimate draft manuscript (go to MyCHS to edit an existing record)
• Submit a non-standard penultimate draft manuscript (create a new record for a follow-up paper, or when no proposal was required)
• Update a previously submitted penultimate draft manuscript or its journal status (go to MyCHS). To learn more about modifications, click here.

Abstract or Presentation:

• Notify CHS of an accepted abstract or presentation (go to MyCHS to update the corresponding paper proposal)
